ADLEX Dental Candidate and ADHLEX Dental Hygiene Candidate Manuals and Applications
The ADLEX Examination is given in two formats: the Curriculum Integrated Format (CIF) and in the Traditional Format. The CIF Format can only be taken by students about to enter their senior year of dental school or graduate students at a school hosting the CIF Format examination. The Traditional Format is for those students who elect not to take the CIF Format and for those who have already graduated from dental school. Only students or graduates of schools accredited by the ADA Commission on Dental Accreditation or the Commission on Dental Accreditation of Canada may take the ADLEX Examination. All other candidates must apply through the state dental board in the state where they wish to practice for permission to take an examination for licensure only in that state.
The current examination in dental hygiene offered by NERB IS the ADHLEX Examination approved by the American Board of Dental Examiners. Only dental hygienists who have graduated or are going to graduate (within 45 days of the examination date) from programs of dental hygiene accredited by the ADA Commission on Dental Accreditation in the United States or graduates of accredited programs of dental hygiene in Canada may apply for this examination. All other candidates must apply through the state dental board in the state where they wish to practice for permission to take an examination for licensure only in that state.
Examinations are available in Local Anesthesia and Nitrous Oxide Monitoring for dental hygienists. Applicants must have graduated or are going to graduate (within 45 days of the examination date) from a dental hygiene program which is accredited by the American Dental Association Commission on Dental Accreditation (ADA/CODA) or by the Commission on Dental Accreditation of Canada (CDAC). Candidates need to check with their state dental board to determine if such credentials are available in their state.

- a foreign trained Dentist or Dental Hygienist
- interested in taking a NERB administered examination for licensure in a particular state
- interested in taking the Massachusetts-only examination in dental hygiene for dental students
- interested in a specialty examination
There are several ways to obtain a candidate manual and application packet:
- If you are entering your senior year at a dental school located in the northeastern U.S., manuals and applications for the CIF Format of the ADLEX Examination will be sent to your school in June and you will be able to obtain them from your Dean’s Office.
- If you have already graduated from dental school or elect not to take the CIF during your senior year or are from a school outside the northeastern U.S. you can obtain a Traditional Format manual and application by calling 301-563-3300 Ext. 505.
- Alternatively either the dental CIF or the Traditional Format Manual can be downloaded below from this site in Adobe Acrobat format. However, the application is available only as a hard copy through the mail, as it is read by a scanner and any alteration of its dimensions will render it unusable.
- If you are about to graduate from a dental hygiene school in the northeastern U.S., manuals and applications for the ADHLEX examination in dental hygiene will be sent to your school in January and you will be able to obtain them from your Program Director.
- If you have already graduated from a dental hygiene program or are from a program outside the northeastern U.S., you can obtain a Dental Hygiene Manual and application by calling 301-563-3300 Ext. 505.
- If you are a dental hygiene graduate of an accredited program and wish to receive a manual and application for the Local Anesthesia or the Nitrous Oxide Examination, call 301-563-3300 Ext 505. Alternatively the manual can be downloaded below from this site in Adobe Acrobat format. However, the application is available only as a hard copy through the mail, as it is read by a scanner and any alteration of its dimensions will render it unusable.
- You can make an on-line request for any of the manuals using the form located below or you can fax your request to 301-563-3307. Please include your name and address and indicate whether you are interested in a Dental CIF, Dental Traditional, Dental Hygiene, Local Anesthesia or Nitrous Oxide manual. Allow 1 to 2 weeks for delivery. Please make only one request.

If you have graduated from a Dental or Dental Hygiene school or are interested in the Dental Hygiene Local Anesthesia or Nitrous Oxide examination you may make an on-line request.
If you are graduating from a Dental or Dental Hygiene school and your school is located in the northeastern U.S., manuals and appendices will be sent to your school in late January for dental hygiene candidates and in June of your Junior year for dental candidates. You will be able to obtain them from your Dean or Program Director at that time.
